Enrollment Requirements
TK-12TH REQUIRED DOCUMENTS FOR SCHOOL ENTRY
Completing and submitting all required documents promptly during registration will ensure that your child can begin the school year successfully and without any unnecessary delays. If you have any further questions, please contact the academy in which your student will be attending.
Required Documents
- Proof of Age - Birth Certificate, Passport, Affidavit of Parent/Legal Guardian (TK-12th Grade)
- Immunization Records Required Immunizations for School Entry Guide (TK-12th Grade)
- Report of Health Examination (TK/ New Kindergarten and 1st Grade)
- Oral Health Assessment Form (TK/ New Kindergarten and 1st Grade)
- Proof of California Residency (TK-12th Grade)
- Transcripts/Report Cards (9th-12th Grade only)
- Updated Immunization Records (Currently attending NCS students entering the 7th Grade)
Additional Documentation (if applicable)
- Photocopy of current IEP (for students receiving special education services)
- Photocopy of current 504 Plan (for students receiving special education services)
- Photocopy of Court Order
Document Submission Options
All documents are required to be completed in full and uploaded during your students online registration, submitted via email or in-person to the academy that your student is enrolling in. Should you have any issues with uploading your documents, please visit or contact the academy that you are enrolling in and they will be happy to assist you in completing the online registration.
